Job Description

With 118 beds and more than 400 physicians on its medical staff, Pali Momi Medical Center offers a full range of services for the communities of Central and West O'ahu. The hospital has delivered many medical firsts for the community, including Central and West O'ahu’s only interventional cardiac catheterization unit to detect and treat heart disease, a fully integrated minimally invasive surgical suite, a comprehensive women’s center, CT scan and MRI services, and the state’s first retina center. Pali Momi is the only facility in Hawai'i that uses a team triage approach in its emergency room, where patients are promptly evaluated by an emergency physician.

As the Transitional Care Coordinator, you will oversee and design the most compassionate and sensitive health care plan for patients moving through the Pali Momi care system. This includes leading and developing strategies to assess, manage and prevent re-admissions using case management, nursing expertise and focused planning for patients. You will initiate the clinical management of targeted transitional patient populations by providing educational information in conjunction with direct care providers on: treatments, procedures, medications, and continuing care requirements. You will also build agreement about care objectives; identify progress toward desired outcomes and intervene to overcome deviations in the expected plan of care when appropriate. Minimum Qualifications: Bachelor’s degree and/or equivalent combination of education, training and experience. Must meet education requirements for Hawaii state licensing. Current Hawai‘i Registered Nurse License. Three (3) years acute care nursing experience demonstrating experience in process improvement and quality improvement including core measures.

Preferred Qualifications: Master’s degree in nursing or health related field. Previous experience in case management, transitional care or managed care.

Rate of pay for selected candidates will be determined by various factors including knowledge, skills, abilities, relevant experience and training, as well as internal peer equity.

Starting Hourly Rate: 58.35
